Transforming Community Services: Primary Care Trust Survey


UNISON health group would like to get an update on what is happening in every PCT in England in response to the Transforming Community Services programme. We are requesting that our 12-question survey is completed for each PCT in England. We anticipate that it should take about 10 minutes to fill in.
